Diabetes Prevention Program

Montefiore Nyack Hospital Now Enrolling Participants for January 2024

Montefiore Nyack Hospital will offer a Diabetes Prevention Program for adults 18 or older who have prediabetes or are at risk for type 2 diabetes. Recruitment is underway for interested participants who meet the eligibility criteria for this free program. Participants will meet once a week with a trained lifestyle coach from the Hospital’s Community Health and Wellness Department to improve their food choices, increase physical activity and learn coping skills to promote weight loss and reduce the risk of developing type 2 diabetes. 

Developed in partnership with The National Diabetes Prevention Program in New York State, Montefiore Nyack Hospital’s Diabetes Prevention Program is recognized by the Centers for Disease Control and Prevention (CDC) and aims to make it easier for people with prediabetes to participate in evidence-based, affordable and high-quality lifestyle change programs to reduce their risk of type 2 diabetes and improve their overall health. 


Research studies have shown that this structured lifestyle change program, in which participants achieved a weight loss of 5 to 7 percent of their body weight (10 to 14 pounds for a person weighing 200 pounds), reduced the risk of developing type 2 diabetes by 58 percent in adults who are at high risk for the disease. 

According to the CDC, 37.3 million Americans—about 1 in 10—have diabetes. Every year, about 1.4 million new cases of diabetes are diagnosed. For people aged 10 to 19 years, new cases of type 2 diabetes increased for all racial and ethnic minority groups, especially Black teens. Diabetes was highest among Black and Hispanic/Latino adults, in both men and women. 

Approximately 96 million U.S. adults—one out of three—have prediabetes, and 80 percent—eight out of 10—of them don’t even know they have it. Prediabetes is a condition in which blood glucose or hemoglobin A1C (HbA1C) levels are higher than normal but not high enough yet to be diagnosed as type 2 diabetes. Without intervention, 15 percent to 30 percent of people with prediabetes will develop type 2 diabetes within 5 years. People with type 2 diabetes have an increased risk of heart attack and stroke.

About the National Diabetes Prevention Program

The National Diabetes Prevention Program—or National DPP—was created in 2010 to address the increasing burden of prediabetes and type 2 diabetes in the United States. This national effort created partnerships between public and private organizations to offer evidence-based, cost-effective interventions that help prevent type 2 diabetes in communities across the United States. The National DPP is a research-based program focusing on healthy eating and physical activity, which showed that people with prediabetes who take part in a structured lifestyle change program can cut their risk of developing type 2 diabetes by 58% (71% for people over 60 years old).
