At times, more care is needed than what a caring family member is able to provide. For these individuals, Nyack Hospital’s Long Term Health Care Program may provide the answer. Known as the “Nursing Home Without Walls,” this service offersan alternative to nursing home placement for individuals who wish to remain in the comfort of their own home, by providing and coordinating a comprehensive range of health, social and environmental services. Nursing case managers work to meet the unique and diverse needs of the frail elderly, chronically ill or disabled person.

Medical Social Work Services for Patient/Family Counseling
Physical Therapy
Occupational Therapy
Speech Therapy
Home Health Aides/ Personal Care Aides
Housekeeping Services
Lifeline Personal Emergency Response Services
Meals on Wheels
Social Day Care
In-home or In-patient Respite Care
Medical Supplies/Equipment
24-Hour On-Call Nursing Services
Eligibility
Patients are eligible for the Long Term Care Program if they:
Reside in Rockland County
Have physician approval
Are currently covered by Medicaid
Are medically eligible
Require services not exceeding 75% of the average annual cost of a skilled nursing facility or an adult home located in the patient’s area of residence.
Are approved by the Rockland County Department of Social Services
